Your First Dental Visit
If you’re visiting us for the first time for a dental checkup and cleaning, we ask that you come with your new patient forms already filled out to save you some time. We’ll input your information into our system and guide you to one of our comfortable treatment rooms. There, you’ll have a chance to meet our dental hygienist who will take X-rays of your mouth. Dr. Shively will conduct a thorough examination of your teeth and gums, and finally, we’ll finish your visit off by scaling, polishing, and flossing your teeth.
New Patient Forms
If you’re new to our dental office, we ask that you download and fill out our forms at home before you visit us. This way, you don’t have to spend any time fumbling for your insurance card or other important information in our waiting room and we can see you immediately.
Dental Insurance Welcome
We’re happy to file our patients’ insurance claims on their behalf. We’ll do our best to maximize your benefits and always make sure to explain any fees or payments that you’ll be responsible for when you schedule an appointment.
